Application Form

Under the Law on the Protection of Personal Data No. 6698 (“LPPD”), personal data owners, defined as data subjects, or their legal representatives (“Applicant” or “Data Subject”), are granted rights to make requests concerning the processing of their personal data as regulated in Article 11 of the LPPD.

In accordance with Article 13/1 of the LPPD, applications concerning these rights to the data controller, AloTech İletişim Teknolojileri Ticaret A.Ş. (“AloTech”), must be submitted in writing or through other methods determined by the Personal Data Protection Board (“Board”). Depending on its nature, your request will be answered as soon as possible and within thirty days at the latest from the date it reaches AloTech.

1. Application Method

In accordance with Article 13/1 of the LPPD, the following methods can be used for applications to be made to the data controller, AloTech, regarding requests concerning the processing of personal data:

• Application by Post or In Person: The Applicant can apply in person or by post to the address “NidaKule Plaza Kozyatağı Mahallesi, Değirmen Sokak, No:18 KAT:13 D:22 Kadıköy İstanbul” with the completed and signed application form and documents verifying their identity.
• Application via Registered Electronic Mail (KEP): An application can be made by sending an email from the Applicant’s registered electronic mail (KEP) address to alotech@hs01.kep.tr with a signed petition or, if preferred, the “Application Form”.
• Application via Email Using Mobile Signature or Secure Electronic Signature: An application can be made by sending an email to alotech@hs01.kep.tr with a petition signed by the Applicant with a mobile signature or secure electronic signature, or by filling out the “Application Form”.
• Application by Email: An application can be made by sending an email to kisiselverilerim@alo-tech.com using the email address previously notified to the data controller AloTech by the Applicant and registered in AloTech’s system. In this application, AloTech reserves the right to request additional identity verification depending on the subject of the request.

Applications can also be made through a Notary Public or other legally valid methods that provide identity verification. It is recommended to comply with the above-mentioned points to the extent permitted by the chosen method, as this will increase the likelihood of a positive and prompt outcome for the application.

• I want to know if my personal data is being processed.
• If my personal data has been processed, I request information about it.
• I want to learn the purpose of processing my personal data and whether it is used in accordance with its purpose.
• I want to know the third parties to whom my personal data is transferred domestically or abroad.
• I request the deletion, destruction, or anonymization of my personal data within the framework of the conditions stipulated by law.
• I request the correction of my personal data because it has been processed incompletely or incorrectly. (Please provide detailed information in the explanation section about the personal data you want to be corrected).
• I request that if changes are made to my personal data upon my request, these changes be notified to the third parties to whom my personal data has been transferred.

Method of Response to Your Application

(If no response preference is selected, the response will be sent using the same method as the application.)

• I want it to be sent to my address. (By delivery to the address specified in the application.)
• I want it to be sent to the email address I specified in the application form. (We will be able to serve you faster if you choose the email method.)
• I want to pick it up in person. (In case of collection by proxy, a notarized power of attorney or authorization certificate is required.)

This Data Subject Application Form has been prepared to ensure that your requests can be answered correctly, completely, and within the time specified by law. AloTech reserves the right to request additional documents and information (such as a copy of your ID card or driver’s license, etc.) for identity and authorization verification to eliminate legal risks that may arise from unlawful or unjust data sharing and especially to ensure the security of your personal data. AloTech does not accept liability for such requests in cases where the information regarding your requests submitted within the scope of the form is not correct and up-to-date, or if an application is made with false/misleading information or without authorization. In such cases, AloTech reserves the right to reject the application by explaining the reason.
